Girls' Volleyball Registration Form

Grades 3 - 8

Due Date: May 17, 2018

To register your student, please complete the following:

1)  Read the Elementary & Jr. High Sports Policies for Grades 1- 8 and the Parent Code of Conduct found on the school's website.  All students and parents must agree to the Policies and Code of Conduct.

2)  Complete the registration form below.  Please complete one form per child.

3)  Send in the $100 fee per student to participate. Please make your check payable to VMA Athletics and send your check to the main office in an envelope labeled VMA Volleyball.  Please note - registrations cannot be accepted after the due date.

4)  Please note, volleyball practices begin in July.  Games begin in August. Our teams play in various leagues throughout the area.  You will receive an email with more information from your child's coach closer to the start of the season.  Coaches are selected by the Athletic Department.

 My child has a recent physical and is fit to participate.
 I have the necessary insurance to cover any accident or injury that may occur at volleyball and will not hold Villa Madonna Academy or the coaching staff responsible for accident or injury.
 My student athlete promises to follow instructions and abide by the rules given to me by the coach.
 As a parent of a VMA athlete, I will abide by the Parent Code of Conduct.
 I will attend the Parent Meeting held at the beginning of the season to be informed of the VMA Sports Policy, rules and guidelines and sports pledges.
 I will submit the registration fee by May 17.
 Grades 7&8 students must complete and submit the HS KHSAA / PHYSICAL 4 page form found under HS Sports on the website once per year and submit with this form to the Athletic Department in order to participate.

Coaches:

Coaches with volleyball experience are welcome to assist with our programs. Complete the information below if you would like to volunteer. All coaches must be current on all Virtus bulletins. A coach meeting will take place in September. All coaches are required to attend and submit the Rules for Coaching and Pledge form. A current CPR AED / First Aid certificate is required. More information on additional classes will be available soon.
